Diamond IV Therapy LLC | Connecticut. These terms describe use of diamondivtherapy.com and requests for Diamond IV Therapy services. Separate treatment consent, clinical instructions, and the specific plan terms shown at purchase also apply. Website information is general service information, not a diagnosis, individualized medical advice, or a guarantee of results. Treatment requires clinical screening and any necessary provider approval and prescription. A request or purchase does not guarantee a specific treatment is appropriate. Your provider determines eligibility and treatment. Medical weight-loss services require a licensed provider's evaluation; medication, dose, availability, outcomes, and ongoing care are not guaranteed. Review program duration, inclusions, laboratory charges, and follow-up costs before enrolling. Results vary.

2. APPOINTMENTS AND MOBILE VISITS

Online appointments are requests until confirmed by Diamond IV Therapy. We confirm clinical eligibility, your address, visit arrangements, and any travel charge before the visit. Provide accurate contact and location information and contact us about changes before your appointment. A website inquiry alone does not establish a treatment relationship. Travel is measured in driving miles from West Haven, Connecticut, per visit: up to 20 miles is included; 21–40 miles adds $50; over 40 miles adds $100. These travel terms apply to individual treatments, memberships, and packages. Standalone injection visits require a $199 minimum appointment total, plus any applicable travel fee. Availability and visit arrangements must be confirmed before service.

3. PAYMENTS, PLANS, CANCELLATIONS AND REFUNDS

Review your service price, selected plan terms, and confirmed travel or additional charges before proceeding. Payment methods and arrangements are confirmed during booking; this website does not guarantee a particular payment method or insurance coverage. Membership renewal, billing interval, cancellation notice, included visits, and credit expiration follow the specific plan terms shown at purchase. Package validity and included services follow the selected package terms. Mention your membership or package when booking so we can confirm available credits. Cancel or reschedule using your booking link or by contacting 203-709-1403 or crystal@diamondivtherapy.com as soon as possible. Membership cancellation is separate from canceling a visit and follows your plan terms. Contact us about refunds, billing errors, unused packages, or clinical ineligibility before treatment. Disclosed purchase refund rules and your rights under applicable law apply. These website terms do not create a new cancellation fee or blanket no-refund rule. Existing purchases remain subject to their agreed terms; future changes require any notice or consent required by law or the purchase terms.

4. WEBSITE USE, EMERGENCIES AND CONTACT
